- comments: observed no visible thermometer in walk in cooler or walkin freezer. Instructed to provide visible thermometer in walkin cooler and walkin freezer.

- comments: observed dead waterbugs throughout basement. Instructed to remove dead waterbugs and clean affected areas.

- comments: observed boxes, shelves, racks and pallets stored on floor in basement storage rooms and boxes of nonfood items stored on floor in retail and in rear storage. Instructed to provide adequate shelving at least six inches off of floor for ease of cleanability and to prevent areas for pest harborage.

Observed black moldlike substance and spillage on interior shelving and base of multi-door cooler in rear retail area. Observed interior of shelves of non working cooler with black moldlike substance. Observed dust on exterior of all small cold holding units in retail. Instructed to detail clean interior and exterior of all cold holding units and maintain.

- comments: observed no covered waste receptacle in either unisex washroom. Instructed to provide covered waste receptacles in both unisex washrooms and maintain.
