Food Safety Report - Domino's Pizza, Mount Juliet Tennessee

6 years ago reported by user-wvkzt552 business

Domino's Pizza, Lebanon Road, Mount Juliet, TN, USA

11362 Lebanon Road, Mount Juliet, 37122 Tennessee, United States

On July 21, 2018 (Saturday) I ordered a medium garlic parmesan white sauce pizza with Italian sausage and spinach. I received it around 4:45pm. I ate 5 slices of this pizza. About 20-30 min. later, I started getting major stomach cramps and vomited everything I ate. I have been experiencing: vomiting, diarrhea, cold sweats, chills, nausea, stomach cramps, fatigue, and feeling extremely weak. I'm currently on day two and I still don't feel right. It's Sunday and I'm trying to wait till Monday to see my doctor. | Symptoms: Diarrhea, Nausea, Vomiting, Cramps, Weakness, Chills, Fatigue, Sweating
