1. Person in charge present, demonstrates knowledge, and performs duties
- comments: no person in charge on site during inspection. Priority foundation. No citation issued.

2. City of chicago food service sanitation certificate
- comments: no person in charge on site during inspection with a city of chicago food manager certificate while tcs foods were being prepared and served (espresso). Instructed to have a person on site at all times with a valid and original city of chicago food manager certificate. Priority foundation. 7-38-012.

3. Management, food employee and conditional employee; knowledge, responsibilities and reporting
- comments: observed no employee health policy/training on site. Instructed facility to establish an appropriate employee health policy/training system and maintain with verifiable documents on site. Priority foundation violation. 7-38-010.

5. Procedures for responding to vomiting and diarrheal events
- comments: observed no vomit and diarrhea clean up kit and procedure on site during inspection. Instructed to have a procedure along with the necessary supplies as outlined in policy. Priority foundation. 7-38-005.

8. Hands clean & properly washed
- comments: observed food employee taking an order on the register in front coffee prep area. Immediately after, employee started to prepare an espresso drink. Instructed to wash hands before engaging in food preparation including working with exposed food, clean equipment and utensils. Priority violation. 7-38-010.

10. Adequate handwashing sinks properly supplied and accessible
- comments: observed no employee hand washing signage in bathroom. Must provide.

38. Insects, rodents, & animals not present
- comments: observed no pest control log book on site during inspection. Instructed to provide a pest control log book that includes a contract with a licensed exterminator who will provide insect and vermin services at least twice a year. Service reports must be included along with the illinois department of health pest control license number. Priority foundation. 7-38-020(c).

40. Personal cleanliness
- comments: observed two food handlers preparing food without a hair restraint. Instructed that food employees must wear hair restraints such as hats, hair coverings or nets that cover body hair and are designed and worn effectively to keep their hair from contacting exposed food, clean equipment, utensils and linens.

- comments: observed no visible back flow preventer device on ice machine in kitchen prep area. Must provide.

- comments: observed cooler in rear in disrepair. Must remove from premises. Observed excess clutter in rear dry storage hallway, hallway leading to hot water heater, and office. Must get rid of all unnecessary items/equipment and store all remaining items at least 6 inches off floor.

- comments: observed no food handler certificates on site. Instructed food handlers to complete food handler training required by the state of illinois.

- comments: observed no allergen training certificates on site. Instructed to provide allergen training for all employees who obtain the city of chicago food manager certificate.
