- comments: procedure/plan for responding to vomiting and diarrheal events on site but has in-complete clean-up items. Instructed to provide and maintain required procedure/plan and appropriate clean-up items or kit. (necessary items at a minimum: gloves, face masks, disposable mops and appropriate sanitizer/spray that is effective against norovirus). Priority foundation violation 7-38-005.
40. Personal cleanliness
- comments: observed manager preparing protein shakes and teas without wearing a hair restraint. Instructed management to provide hair restraint when preparing open beverages.
51. Plumbing installed; proper backflow devices
- comments: backflow prevention device not located at ice machine. Must install so backflow prevention device may be located to be serviced and maintained.
54. Garbage & refuse properly disposed; facilities maintained
- comments: observed outside garbage receptacle not maintained;overflowing with trash and lids wide open with trash above rim. Instructed to notify garbage disposal co. For service and maintain at all times. Lids/cover must be kept closed to prevent pest harborage. Priority foundation violation 7-38-020 (b),citation issued.

- comments: instructed to remove accumulations of dusts on ventilation cover in unisex washroom.
