Approximately 37 live flies observed in establishment: 29 in back dry food storage area near main prep area, two on main cook line in kitchen and six by restrooms in dining area. **Repeat Violation** **Admin Complaint** - From follow-up inspection 2021-09-17: Observed five live flies in kitchen. **Admin Complaint** - From follow-up inspection 2021-09-17: No live flies observed - 35A-05-4: Roach activity present as evidenced by live roaches found. Inspector observed one live roach on wall back of soda machine at front counter. Roach killed and discarded. **Admin Complaint** **Corrective Action Taken** - From follow-up inspection 2021-09-18: Observed seven live flies in kitchen area.
