- comments: the person in charge has no city of chicago food service managers certificate as required; instructed to obtain. Priority foundation violation 7-38-012 citation issued.

- comments: observed the following in need of cleaning: bottom area and exterior surfaces of fryers, storage rack for packaged bread, odor inside of walk in cooler. Clean and maintain all.

51. Plumbing installed; proper backflow devices
- comments: leaking observed from pipe underneath the first compartment; repair leak at pipe.

- comments: found unnecessary items in unused walk in unit; instructed to remove to avoid clutter. Found unnecessary items in utility sink; instructed to remove and not obstruct sink.
