- comments: observed no handwashing sign at handwashing sink in kitchen. Instructed manager to provide handwashing signs at all handwashing sinks used by employees.

- comments: observed over 100 mice droppings scattered on cardboard boxes and on floor under shelving in dry food storage area, inside cubbies, on floor along walls and corners in room 100, on floor along walls and on shelves in pre-k 105 and 106. Instructed manager to call an exterminator for service, clean and sanitize all areas. Priority foundation 7-38-020(a)

- comments: observed 1/4" gap at botttom right corner of exit door in room 100. Instructed manager to seal all outer openings to prevent pest entry.
54. Garbage & refuse properly disposed; facilities maintained
- comments: observed outside middle dumpster missing lids and not covered. Instructed manager to replace missing lids and cover all outside dumpsters. Priority foundation 7-38-020(b)
