- comments: observed no soap at hand washing sinks in 3rd floor boys north and south washrooms. Instructed manager to provide soap at all hand washing sinks for proper hand washing. Priority foundation 7-38-030(c)

- comments: observed timed faucets not operable in boys 2nd floor washroom in old building, left faucet in girls 3rd floor washroom in old building and new building, boys 4th floor washroom in old building. Instructed manager to repair timed faucets, water must run continuously for at least 15 seconds before reactivating faucet.

- comments: observed over 100 mice droppings scattered on floor along walls and corners in closet and washroom in pre-k room 103, over 30 mice droppings scattered on floor along walls and corners in pre-k room 104, over 10 mice droppings scattered on floor and shelves in pre-k room 102, 5 mice droppings scattered on floor in corner near desk in pre-k room 101, over 20 mice droppings scattered on floor along walls near booths, vending machine, and entrance door in faculty lounge 112. Intructed manager to call an exterminator for service, clean and sanitize all areas. Priority foundation 7-38-020(a)

- comments: observed the following doors not rodent proofed, 1/4" gap at bottom of kitchen exit door, 1/4" gap along door frame at exit #6, 1/4" gap at bottom corners of exit #2 and exit #3, 1/4" gap at bottom of all three northeast exit doors in old building. Instructed manager to repair and seal all outer openings on doors to prevent pest entry.

- comments: observed no running water at right hand washing sink in boys first floor washroom north, right clogged drain at hand washing sink in girls 2nd floor washroom in old building, left clogged drain at hand washing sink in girls 3rd floor washroom near room 306. Observed missing cap on second faucet in girls 3rd floor south washroom. Instructed manager to repair and maintain.

53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: observed no covered receptacles in 1st, 2nd, 3rd, and 4th floor girls and staff washrooms in old building. Instructed manager to provide covered receptacles in all girls and unisex washrooms.

55. Physical facilities installed, maintained & clean
- comments: observed openings in ceiling around pipe fittings in pre-k storage 103b. Observed water damage in ceiling of boys 2nd floor washroom, damaged bases on walk-in cooler and walk-in freezer in kitchen. Instructed manager to repair and maintain.
