- comments: 6-501. 111 observed evidence of rodent activity on site. Observed about 30 rodent droppings on the dry storage shelves and behind reach in freezers located in the food prep area as well as about 30 rodent droppings in the utility closet and about 5 rodent droppings in the toilet room. Instructed to eliminate rodent activity, clean and sanitize affected areas and recommended contacting pest control for a service. Priority foundation. Citation issued 7-38-020(a).

- comments: 6-202. 15 observed the front entrance door with a 1/4 inch gap at the bottom. Instructed to make said door tight fitting and rodent proofed.
40. Personal cleanliness
- comments: 2-402. 11 observed the food service sanitation manager prepping food without a hair net and beard guard. Instructed to provide for all food handlers.
45. Single-use/single-service articles: properly stored & used
- comments: 4-502. 13 observed cardboard being re-used as a shelf liner on the dry storage shelves. Instructed to remove and do not re-use cardboard, all surfaces must be smooth and easily cleanable.
54. Garbage & refuse properly disposed; facilities maintained
- comments: 5-501. 15 observed the outside grease dumpster not maintained with excessive accumulated grease on the lid and the lid not tight fitting. Instructed to remove grease and ensure the lid stays tight fitting at all times. Recommended contacting the grease dumpster company to get the grease dumpster lid replaced. Priority foundation. Citation issued 7-38-020(b).

- comments: 2-102. 13 observed a male food handler without proof of food handler training. Instructed to meet requirement for all food handlers.
