- comments: still observed evidence of appx. 1- rat droppings inside of fryer cabinet,1- live roach at crevices of rear prep counter and a dead rat in motor of rear unused reach in cooler. . Must remove rat dropping& dead rat clean and sanitize areas. Must have a pest control operator to service premises on or after 4/14/25. (priority foundation 7-38-020(a)(citation issued)

- comments: 6-202. 15 : observed a gap along bottom of rear door(north) make doors tight fitting to prevent insect entry.
