- High Priority - Roach activity present as evidenced by live roaches found. Observed two live roaches crawling inside of the kitchen reach in cooler, three live roaches crawling inside of the walk in cooler, one live roach behind back area reach in cooler.

- High Priority - Stop Sale issued on time/temperature control for safety food due to temperature abuse. Observed Raw beef (68 F - Cold Holding); Raw Pork (68 F - Cold Holding) Cooked with rice (68 F - Cold Holding); according to operator stored in walk in cooler since yesterday . Observed Front prep area Sliced tomatoes (60F - Cold Holding); Sliced Ham (58 F - Cold Holding); Sliced pork (59 F - Cold Holding); American cheese (59 F - Cold Holding)

- High Priority - Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed Raw beef (68 F - Cold Holding); Raw Pork (68 F - Cold Holding) Cooked with rice (68 F - Cold Holding); according to operator stored in walk in cooler since yesterday .

- High Priority - Toxic substance/chemical improperly stored. Observed a WD40 oil spray on top of the three compartment sink.

- High Priority - Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet.
