- comments: observed no city of chicago certified food manager on premise during time of inspection with valid original certificate posted for public view. The facility is operating. Preparing and serving (milk, sausages, mashed potatoes and gravy). Instructed to provide and maintain at least one certificate holder on site at all times. Priority foundation violation. #7-38-012. Citation issued.

- comments: observed no metal stem themometer on the premise for testing the internal temperature of food. Instructed to provide and maintain on-site. Priority foundation violation. #7-38-005. Citation issued.
38. Insects, rodents, & animals not present
- comments: observed approximately 25 small rodent droppings on the floor in the prep area alongside refrigerator and hot hold unit, in 2 of 3 of the toilet rooms in corners around the toilet and washbowls. Instructed to remove droppings and clean and sanitize said areas. Must eliminate pest activity on the premises. Priority foundation violation. #7-38-020 (a). Citation issued.

- comments: no chemical test strips were available on site during the time of inspection for testing the sanitizer strength at the 3-compartment sink. Instructed to provide and maintain on-site at all times. Priority foundation violation. #7-38-005. Citation issued.
53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: observed in use toilet room without self closing door. Instructed to provide and maintain in good condition.

- comments: observed room with mop sink in place in need of organizing, removing of all unneccesary items and elevation of all items off floor at least 6 inches. Must mantain premise.
