- comments: observed the center of the door in the center of the facility not rodent proofed at the bottom as required. A mail slot is located at the bottom of the door for mail and is not closed entirely allowing possible rodent entry. Must rodent proof door and maintain.

- comments: observed three dead mice on glue boards in the basement area in furnace closet and outside of furnace closet; instructed to remove.

- comments: observed rodent activity on premises. Found 80 or more mouse droppings in the basement area on the floor in the furnace and in storage rooms, in the kitchen by the rolling cart next to the stove, under the sinks, under the refrigerator, along side of the refrigerator and behind it. Also observed droppings in two classrooms by the windows and along the wall base in the center classroom and the classroom where the basement door is located. Instructed to clean all areas and have pest control company service prior to reinspection next week. Priority foundation violation 7-38-020(a), citation issued.

- comments: observed the water squirting from the faucet when turned on in the washroom closest to the kitchen; instructed to provide a washer for the faucet.
53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: found no self closing device on the adult washroom door; instructed to install a self closing device.
55. Physical facilities installed, maintained & clean
- comments: observed several water stained ceiling tile; instructed to replace where needed. Observed the lower wall in the third classroom in poor repair near the back exit door due to a leak from above; instructed to repair. Observed the wall base in the basement area not completely sealed between the wall and the floor leaving a space for rodent to enter; instructed to seal. Observed a hole in the floor of the kitchen in the corner area under the exposed handsink; instructed to seal.
55. Physical facilities installed, maintained & clean
- comments: instructed to clean floors in all areas where dropping observed and dead mice. Observed dust on the ventilation fan on the ceiling of the adult washroom; instructed to clean to remove dust.
