- comments: instructed manager that any food handlers in the food prep area must wear a proper hair restraint.

55. Physical facilities installed, maintained & clean
- comments: observed the wall base in poor repair in the rear next to the pop syrup storage area. Also noted standing water in same area. Must repair and maintain to prevent breeding point for pest.
