Observed 1 live flying insect on three compartment sink Observed 29 live flying insects under three compartment sink. Observed 5 live flying insects under prep sink in back storage. Observed three live flying insects by mop sink Observed 2 live flying insects on cases of cups in back dry storage. Observed 1 live flying insect on paper towel hanging from dispenser. **Repeat Violation** - From follow-up inspection 2021-07-08: Observed 2 live flying insects on wall left to back door. Manager eradicated and cleaned and sanitized area. Observed 1 live flying insect on container of fryer oil. Manager eradicated. Observed 1 live flying insect on picture in lobby Observed 1 live flying insect on wall at make station. Manager eradicated and cleaned and sanitized area - From follow-up inspection 2021-07-09: Observed 1 live flying insect on wall above back prep table. Manager eradicated, and cleaned and sanitized area. Observed 1 live flying insect flying around case of oil under prep shelf in back. Manager was able to eradicate it. Observed 1 live flying insect on drain pipe under three compartment sink. Manager eradicated and cleaned and sanitized area. Observed 1 live flying insect flying around cleaned and sanitized pans in back prep area. Manager eradicated it. Observed 2 live flying insects flying around hot stam well in by front counter.
