- comments: observed 30 mice droppings scattered through out musical instruments and on floor in the music room. Instructed to detail clean and remove droppings. Priority foundation violation #7-38-020(a).

- comments: : found the unisex and women's staff toilet rooms with trash receptacles with no lids/covers. Instructed to provide receptacles with lids/covers.

54. Garbage & refuse properly disposed; facilities maintained
- comments: observed the outside dumpster top loaded allowing possible food source for pests. Also detail clean and maintain the area around the dumpster. Priority foundation violation #7-38-020(b).

55. Physical facilities installed, maintained & clean
- comments: observed peeling paint in the kitchen above the prep table and in the dining area on the vents. Must remove paint chips and repaint.
