- comments: observed the wall and baseboard area in the side storage/hot water tank area on first floor to be rough and not easily cleanable. Various gaps present and surface is uneven. Instructed to repair, replace, and provide a smooth and easily cleanable wall surface.

- comments: observed build up of debris and pieces of broken wall board on the floor behind the hot water tank in the side storage on the 1st floor. Instructed manager to detail clean floor to remove and maintain free of excessive build up.

- comments: observed lever of paper towel dispenser broken on dispenser in women's bathroom. 2 dispensers are available at this time. Instructed manager to service and maintain paper towel dispenser in good repair.

Must install a washable splashguard. #47) 4-501. 11 - noted ice build up inside upright freezer on shelves. Instructed to defrost, clean and maintain. Priority foundation violation: 7-42-090, citation issued.
