Noted several employees without effective hair restraints. Must provide effective hair restraints and beard guards for employees. #49. Top of the dish washing machine extremely dirty with dirt and debris. Must clean and maintain. #55. Floor drains throughout prep areas and under dish machine dirty, stained with dirt and food debris. Floors throughout prep areas, under rear dry food storage, under rear ice machine, throughout walk-in cooler with dirt and debris. Noted the backsplash wall at the three compartment sink and attached faucet extremely dirty. Must clean all and maintain. #55. Ceiling vent cover not properly attached in the women's washroom. Must replace. #38. No proof of allergen training or certificate for the above city of chicago certified food manager. Must provide. Priority foundation 7-42-060. Citation issued.
