High Priority - Live, small flying insects in kitchen, food preparation area, food storage area and/or bar area. Observed 5 flies on wall over three compartment sink. Observed more than 5 flying around in prep area room. Observed more that 5 flying around three compartment sink.
High Priority - Roach activity present as evidenced by live roaches found. 7 live coming from wood base in prep area room. 1 live near reach in cooler in prep area room. 3 live inside reach in cooler. Per manager reach in cooler is not being used. Observed no TCS food and it's unplug. 3 live coming from behind the shelve on wall in prep area. 1 live on door frame going into storage room.
High Priority - Food/ice received from unapproved source/no invoice provided to verify source.
High Priority - Shell eggs not stored at an ambient air temperature of 45 degrees Fahrenheit or less.
High Priority - Stop Sale issued on time/temperature control for safety food due to temperature abuse.
