- High Priority - Employee failed to wash hands before putting on gloves to initiate a task working with food. Observed chef touch cracked eggs, phone and then put gloves on without washing hands

- High Priority - Pesticide/insecticide labeled for household use only present in establishment. Observed raid stored on shelf of preparation table at front line

- High Priority - Raw sewage on ground of establishment. Observed when washing hands sewage water began overflowing from drain line onto floor of establishment at front counter. Also observed in back of kitchen near three compartment sink and dish machine.

- High Priority - Roach activity present as evidenced by live roaches found. Observed 1 live roach crawling on preparation table near mixer in kitchen area. Observed 8 live roaches crawling on preparation table in kitchen area near cooked potatoes.

- High Priority - Shell eggs in use or stored with cracks or broken shells. See stop sale. Observed 2 cracked shell eggs in RIC at front line. Stop sale issued.

- High Priority - Stop Sale issued on time/temperature control for safety food due to temperature abuse. Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed Beef (51F - Cold Holding). As per operator, it was prepared Saturday. Observed Roast Beef (48 - Cold Holding) as per operator, it was prepared Saturday. Observed beef patty (48F -Cold Holding). Butter with garlic (64F - Cold Holding) As per operator, it was placed there the previous day.

- High Priority - Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed Beef (51F - Cold Holding). As per operator, it was prepared Saturday. Observed Roast Beef (48 - Cold Holding) as per operator, it was prepared Saturday. Observed beef patty (48F -Cold Holding) and Butter with garlic (64F - Cold Holding). As per operator, it was placed there the previous day. Stop sale issued.

- High Priority - Toxic substance/chemical improperly stored. Observed sanitizer bucket stored on the floor in back of kitchen. Observed sanitizer bucket stored on preparation table at cook line.
