- comments: observed no city of chicago certified food manager on site during time of inspection while preparing and serving chicken, burgers, italina beef, gyros. Instructed to provide at least one on site at all times and maintain. Priority foundation violation. #7-38-012. Citation issued.
36. Thermometers provided & accurate
- comments: missing thermometers in all cooling units on the premises. Instructed to provide and maintain.
38. Insects, rodents, & animals not present
- comments: observed evidence of pest activity on site. . . Noted approximately 12 small black live flies fling throughout front and rear prep areas and on exterior equipment surfaces. Approximately 100 small rodent droppings on the premises in the following areas. . . Front prep area scattered on floor next to breading station and underneath 2 door cooling unit, underneath all fryers and alongside windowsill front prep, rear prep/dishwashing area underneath 3 compartment sink, rear storage under shelving units, along both sides mop sink. Instructed to remove droppings, sanitize affected areas, and contact pest control for service. Must eliminate pest activity on site. Priority foundation violation. 7-38-020 (a). Citation issued.

- comments: observed no chemical test strips on site during the time of inspection for testing sanitizing solution strength at the 3-compartment sink. Instructed to provide. Priority foundation violation. #7-38-005. Citation issued

- comments: 4-601. 11(c)observed all heavy equipment on the premises with excessive amounts of dried food debris and food splatter on interior and exterior surfaces. Instructed to clean and maintain free of accumulated food residue.

- comments: observed no covered receptacle in unisex toilet room. Instructed to provide.

- comments: observed excessive trash, food debris, flour and grease debris on floors throughout front prep area under all heavy equipment. Also noted pooled blood and chicken pieces on walk in cooler floors. Instructed to remove, clean and maintain.
