- comments: observed timed faucets not operable in boys, girls and unisex washrooms. Water must run continiously for at least 15 seconds before reactivating faucet.

- comments: observed 1/4' gap at bottom of fire exit doors near auditorium and exit doors near room 145 in a building. Instructed manager to seal all outer openings on doors to prevent pest entry.

- comments: observed single service articles stored on floor in dry storage area. Instructed manager to elevate all single service articles six inches off floor.

- comments: observed no hot water at right sinks in thorp second floor boys washroom. Instructed manager to repair and maintain.

53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: observed no covered waste receptacle in thorp and a building unisex washrooms. Instructed manager to provide and maintain.

55. Physical facilities installed, maintained & clean
- comments: observed ceiling leak in kitchen and kitchen office area. Observed water damage and peeling paint on ceiling in kitchen office and thorp second floor hallway. Observed mold-like substance on walls and missing baseboards in dry storage area. Instructed manager to repair/replace and maintain.

Pre-k 124- damaged ceiling. Must remove rust from walk in cooler floor.
