- comments: noted no hand wahing signs at the hand washing sinks. Instructed to put hand washing signs at all hand wash sinks.

36. Thermometers provided & accurate
- comments: noted no thermometers inside refrigeration unit conspicuously posted to monitor the ambient temperature of equipment. Instructed to equip all refrigeration units with accurate and working thermometers.

37. Food properly labeled; original container
- comments: noted bulk food items stored inside containers at the basement food prep/dry storage not properly labeled. Instructed management that all bulk and working food ingredient containers must be labeled with contents name.

38. Insects, rodents, & animals not present
- comments: noted over 20 rat droppings on floor along wall base at the dry storage area. Instructed to detail clean and sanitize affected areas, eliminate pest activity. Instructed to consult with pest control company and have premises treated. Priority foundation #7-38-020(a).

- comments: noted dirty wiping cloths on prep tables at rear prep area not properly stored. Instructed to properly store all washcloths in a sanitizing solution especially when not in use

- comments: cutting boards with open seams and dark stains. Must refinish or replace damaged or stained cutting boards.

- comments: noted rusty and dirty shelves inside refrigeration units at the kitchen prep area. Instructed to detail clean, repaint with non toxic food grade paint and/or replace.

Instructed to detail clean, repair cracks and holes and sanitize especially areas with pest activity, clean and maintain walls with cracks and spills. Must upgrade housekeeping.

- comments: noted heavy clutter throughout establishment on shelves, floors inside unused equipment and in storage areas. Instructed to detail clean, organize and remove all unnecessary or unused articles and equipment to prevent hiding places for pest. Must upgrade housing keeping.

- comments: noted cooking stoves at the kitchen not under the exhaust ventilation hood system. Instructed to have all cooking stoves and grills under the exhaust hood.

58. Allergen training as required
- comments: noted certified food managers on duty with no allergen training certification. Instructed management that all food service managers are required to have the allergen training certification.
