- comments: observed 3 live roaches in the kitchen near 3 compartment sink. Instructed manager to service all areas affected by pests. Priority foundation violation 7-38-020(a) citation issued

62. Compliance with clean indoor air ordinance
- comments: observed several tables in the common dining area with used ashtrays on them. Ashtrays had used cigarettes in them. Observed employee collecting ashtrays from each table on a cart. Near said employee was a table with several packages of cigarettes. Instructed manager to remove ashtrays from premises and informed manager of clean indoor air ordinance. Citation issued 7-32-020(c)
