- comments: observed no city of chicago certified foodservice manager on premises. Instructed city of chicago certified foodservice manager must be on premises at all times. Priority foundation violation 7-38-012 citation issued

- comments: observed cold tcs food(raw eggs in shell), held at improper temperatures ranging between 50. 5-52. 7f. Instructed all cold tcs food must be held at 41f or below. Corrected on site manager voluntarily discarded and denatured food valued at $40 weighing 4lbs priority violation 7-38-005 citation issued

- comments: observed two door refrigerator at improper ambient temperature of 50. 4f. Instructed all cold holding units must be maintained at 41f or below. Corrected on site held for inspection tag placed on two door refrigerator. Priority violation 7-38-005 citation issued

- comments: observed openings in middle, at base and in between panels of rear exit doors and observed opening at lower right corner of front entrance door. Instructed to seal all outer openings to prevent pest entrance.

Instructed to eliminate rodent activity, remove rat droppings and clean affected areas. Recommended licensed pest control company service facility on or after 6/1/2026. - comments: observed sanitizer compartment drain of three compartment sink stuffed with paper towels. Instructed to provide proper stoppers for three compartment sink.

- comments: observed no hot running water on the premises, observed improper minimum water temperatures ranging between 69. 6-76. 5f. Instructed management to provide and maintain hot running water above 110f at the 3 compartment sink and above 100f at all hand washing sinks. Instructed hot water generation and distribution shall be sufficient to meet the peak hot water demands throughout the retail food establishment. Corrected on site observed hot running water restored before conclusion of inspection, observed water temperatures ranging between 106. 0-128. 5f after restoration. Priority violation 7-38-030(c), citation issued

- comments: observed waste water backing up from lid of grease trap onto floor when draining sanitizer compartment of three compartment sink in prep/dishwashing area. Instructed manager to repair grease trap in prep/dishwashing area for appropriate waste water drainage. Priority violation 7-38-030(c) citation issued.

- comments: observed door of all gender washroom to not be self closing. Instructed to install self closing device on all gender washroom door so that it shall be tight fitting and self closing.

- comments: observed grease on exterior of outside grease container. Instructed to clean exterior of outside grease container and maintain.

- comments: observed stained ceiling tiles throughout facility. Observed stained ceiling tiles in areas such as dining room and in rear prep area. Instructed to replace all stained ceiling tiles throughout facility and maintain.

Observed grease buildup on heater installed in ceiling near dishwashing area. Instructed to remove grease buildup on heater installed in ceiling near dishwashing area and maintain. Observed grease buildup on floors, in corners and along bases of wall underneath and behind all cooking equipment and cold holding units in rear prep area. Instructed to clean floors in said areas and maintain.

- comments: observed unused equipment and containers stored outside space in between rear exit and unused walk in cooler. Instructed to remove all unused or non working equipment that is unnecessary to the operation or maintenance of the establishment.

- comments: observed dust and grease buildup on filters in both hoods in rear prep area. Instructed to clean filters in both hoods in rear prep area and maintain.

57. All food employees have food handler training
- comments: at time of inspection facility provided no proof of food handler training for one food handler. Instructed to provide food handler certificates for all food handlers.
