- comments: observed all necessary items not on premises for complying with procedure available on site when employees respond to vomitting or diarrheal events. Instructed to maintain on premises all necessary items for complying with written procedure when employees respond to vomitting or diarrheal events. Priority foundation violation 7-38-005 no citation issued
22. Proper cold holding temperatures
- comments: observed cold tcs foods held at improper temperatures ranging between 45. 3-56. 3f. Observed cold tcs foods, yogurt(49. 8-56. 3f), string cheese(45. 3-55. 4f), sliced cheese(45. 7-48. 7f), shredded cheese(50. 7-55. 2f) and cream cheese(50. 4-54. 0f) held at improper temperature. Instructed all cold tcs food must be held at 41f or below. Corrected on site manager voluntarily discarded and denatured food valued at $1000 weighing 50lbs priority violation 7-38-005 no citation issued

- comments: observed two door refrigerator in kitchen prep area at improper ambient temperature of 51. 6f. Instructed all cold holding units must be maintained at 41f or below. Corrected on site held for inspection tag placed on two door refrigerator in kitchen prep area. Priority violation 7-38-005 no citation issued
38. Insects, rodents, & animals not present
- comments: observed approximately 70 mouse droppings in facility. Observed mouse droppings in rear dry storage room, old dishroom storage room, classrooms 130, 131, 132 and 133. Instructed to eliminate rodent activity, remove mouse droppings and clean affected areas. Recommended licensed pest control company service facility on or after 5/18/2026. Priority foundation violation 7-38-020(a) no citation issued
51. Plumbing installed; proper backflow devices
- comments: observed no hot water pressure at right handwashing sink in boy's 2nd floor washroom in new building. Instructed to restore hot water pressure at said handwashing sink in said washroom. (note: there are two other properly functioning handwashing sinks in said washroom. ) observed non working toilet in stall four in first floor girl's washroom in new building, observed non working toilet in stall 3 in second floor girl's washroom in new building and observed non working toilet in first stall third floor girl's washroom new building. Instructed to repair non working toilets in said stalls in said washrooms and maintain. (note: there are properly functioning toilets in said washrooms. )
53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: observed no covered receptacle in employee all gender washroom in kitchen, first floor all gender staff washroom in new building and second floor all gender staff washroom in new building. Instructed to provide covered receptacles in said washrooms in facility for sanitary napkins.
55. Physical facilities installed, maintained & clean
- comments: observed dusty ceiling vent covers above shelving unit across from office in kitchen prep area. Instructed to clean ceiling vent covers above shelving unit across from office in kitchen prep area and maintain. Observed dust on oscillating floor fan in kitchen prep area. Instructed to clean floor fan in kitchen prep area and maintain.

- comments: observed dust on filters in hood. Instructed to clean filters in hood and maintain.
