1. Person in charge present, demonstrates knowledge, and performs duties
- comments: observed no person in charge present with city of chicago certified manager certificate.

2. City of chicago food service sanitation certificate
- comments: no city of chicago certified food manager on site or valid documentation on the premises while tcs foods (rice,beef pho soups etc. ) is being handled and prepared. Instructed the person-in-charge that a city of chicago certified food service manager with an original city of chicago sanitation certificate must be on the premises at all times when (tcs foods)are prepared,handled, and/or served. Priority foundation violation 7-38-012, citation issued.

3. Management, food employee and conditional employee; knowledge, responsibilities and reporting
- comments: observed no written employee health policy on the premises. Instructed to provide and maintain signed employee health policy for each employee working on site. Priority foundation 7-38-010, citation issued.

5. Procedures for responding to vomiting and diarrheal events
- comments: no procedure/plan for responding to vomiting and diarrheal events. Instructed to provide and maintain required procedure/plan and appropriate supplies or kit. (necessary items at a minimum: gloves, face masks, disposable mops and appropriate sanitizer/spray that kills norovirus) priority foundation violation 7-38-005, citation issued.

16. Food-contact surfaces: cleaned & sanitized
- comments: observed a slight debris accumulation encrusted on the interior of the ice maker. Must remove debris, detail clean inside ice maker/have serviced and maintain ice machine at all times.

38. Insects, rodents, & animals not present
- comments: observed evidence of pest activity on the premises. Observed approx. 50 rodent mice droppings on the floor in the rear cluttered storage room. Additional pest control service is needed to eliminate the pest activity. Must clean in detail and remove all rodent droppings from affected areas. Priority foundation violation 7-38-020(a), citation issued.

40. Personal cleanliness
- comments: observed employee with no hair restraint in the food prep area. Instructed management to provide hair restraints for all employees in the food prep area.

55. Physical facilities installed, maintained & clean
- comments: instructed to detail clean and organize the rear storage room and remove unnecessary articles due to potential pest harborage and elevate mops,brooms cleaning equipments off the floor at least 6" inches to facilitate cleaning.

56. Adequate ventilation & lighting; designated areas used
- comments: instructed to detail clean the kitchen exhaust hood/filters to remove excessive accumulation of grease build-up and maintain.

57. All food employees have food handler training
- comments: observed no food handler certificates provided on the premises. Must show proof of training for all food handling employees and maintain as required.

60. Previous core violation corrected
- comments: previous core violations not corrected from inspection report#2611029 dated:01/24/2025 #44---4-903. 11 (a;b;d): **c** equipment, utensils, linens and single-use articles- storing observed plates, bowls, pans in main prep area not inverted. Instructed to invert plates, bowls, and pans after sanitizing and air drying. #58---2-102. 13: **c** allergen training - all certified food service sanitation managers employed by a restaurant must receive or obtain training in basic allergen awareness principles within 30 days after employment and every 3 years thereafter. Training programs must be accredited by the american national standards institute or another reputable accreditation agency under the astm international e2659-09 (standard practice for certificate programs). Observed the city of chicago food certified manager with no proof of allergen training certificate. Instructed all city of chicago certified food managers obtain this certificate. Instructed to correct noted violations and maintain by 5/6/2026. Priority foundation violation 7-42-090, citation issued.
