- comments: observed no certified food manager on duty and no certified food managers certificate posted to view while tcs ready to eat foods are being prepared,handled and served such as (italian beef,cooked ground beef,etc). Must be provide and be on site at all times. (priority foundation 7-38-020)

- comments: observed evidence of a live large gray and white cat in rear storage area. 2- open cans of cat food on floor in same area and a open bag of cat litter on floor in prep area. Employee removed cat from premises at this time. Instructed manager no animals are allowed on premises,remove all evidence of a cat on premises. (priority foundation 7-40-060)

- comments: observed evidence of appx. 50 or more rat and mice droppings on floor near exit door in storage area,on sales floor under display shelves and on lower shelf under chip display unit,in walk in cooler on top of excessive caulk along floor and wall. Must remove droppings. Clean and sanitize areas. Recommended to have a pest control operator to service premises on or after 2/19/20. (priority foundation 7-38-020 (a)

- comments: observed an appx. '1/2-3/4' gap along bottom of double front doors. Must make doors tight fitting.

- comments: must store wiping cloths in a sanitizing solution when not in use.

- comments: must repair or replace worn door gasket on milk walk in cooler.
