- comments: observed 10 mouse droppings in facility. Observed mouse droppings on both sides of stainless steel shelf near entrance to basement. Instructed to eliminate rodent activity, remove mouse droppings and clean affected areas. Recommended licensed pest control company service facility on or after 5/28/2026. - comments: observed no service sink in facility. Instructed to install a conveniently located service sink that shall have an adequate supply of hot and cold water under pressure and properly connected to the city of chicago water supply equipped with a floor drain for the cleaning of mops and for the disposal of mop water or similar liquid waste.
