10. Adequate handwashing sinks properly supplied and accessible
- comments: observed hand sink not accessible in rear main kitchen prep area. Observed a plastic bag over faucet,a knife and a large container of dish soap inside of basin and the butcher prep area a grocery cart in front of sink. Articles and cart were removed during inspection. Must keep accessible at all times. (priority foundation 7-38-030(c)

22. Proper cold holding temperatures
- comments: observed improper temperature of 10lbs. Raw grounded pork 46. 4f and 6lbs. Raw steak 45. 4f inside of 2- door prep cooler in main prep kitchen. Product was discarded by manager. Must have cold holding foods at 41. 0f or below. Appx. 16lbs. $40. (priority 7-38-005)(cos)

- comments: must label food storage containers when food is not in original package.

38. Insects, rodents, & animals not present
- comments: observed evidence of appx. 40 or more mice droppings on floor and under produce cooler,on top of box in basement storage room and scattered on top of several blue crates in basement. Also,appx. 15 or more live small flies flying around lights in basement ceiling. Must remove droppings. Clean and sanitize areas. Recommended to have a pest control operator to service premises on or after 5/15/20. (priority foundation 7-38-020)

- comments: must repair or replace worn gaskets on chest freezer in basement.

- comments: observed hand sink in main prep kitchen not operable with a plastic bag over faucet and front service counter prep area faucet in poor repair have to use shut off valve underneath sink to turn water on and off. Instructed to use additional hand sinks on site for hand washing. Must repair and maintain. (priority 7-38-030( c)

- comments: must place cover tightly on sump pump.
