- comments: observed no metal stem themometer on site for testing internal temperatures of food. Isntructed to provide and maintain on site. Priority foundation violation. 7-38-005. Citation issued.

- comments: observed no common food name labels on bulk food containers containing salt, sugar, herbs/spices when items are removed from original packaging. Items stored in rear on shelving units. Instructed to provide and maintain.
47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: observed broken gaskets on upright coco cola refrigeration unit in front prep near grill as well as broken door handle. Must repair and maintain in good condition.
49. Non-food/food contact surfaces clean
- comments: observed excessive dried food debris inside unused refrigeration units in rear storage as well as hot box located in front of facility. Instructed to clean and maintain interior surfaces while not in use.

- comments: observed water stained tiles in ceiliing directly over food prep area as well as front facility. Instructed to replace smooth and easily cleanable ceiling surfaces. Must maintain.

- comments: observed inadequate lighting throughout premise. Instructed to replace burned out light bulbs especially in front prep area where food is prepared, under ventilation hood. Must maintain adequate lighting on site while operating and maintain.
