- comments: observed facility without chemical sanitizers needed to sanitizer equipment and utensils. Must provide chemical sanitizers and be available for use during all hours of operation. Priority violation #7-38-005. Citation issued

- comments: observed rear exit door not rodent proof. Noted 1/4th inch gap northeast corner of door. Instructed to repair and maintain tight seal to prevent pest entry and harboage.
55. Physical facilities installed, maintained & clean
- comments: noted uneven broken floor panels in rear office space. Instructed to restore to a smooth, easy to clean, even surface to prevent pest entry and harborage.
58. Allergen training as required
- comments: observed no proof of allergen training for all certified food managers. Instructed any persons with the city of chicago food sanitation certificate to comply with completion of training and provide and maintain documentation on site at all times.
60. Previous core violation corrected
- comments: noted continued non-compliance from report #2613846 conducted on 3/14/2025. Previous core violations not corrected: (38) front door not rodent/insect proofed. 1/4" gap noted. Instructed to rodent/insect proof front door and maintain. (41) wiping cloths not properly stored when not in use. Instructed to store in sanitizing solution (must be at the required ppm) when not in use. Maintain same. (47) cutting boards with deep, dark stains and grooves. Instructed to either sand or replace cutting boards to maintain a smooth and easily cleanable surface. (49) lower surface of the reach in cooler/freezers with food/dirt. Instructed to clean and maintain coolers/freezers. (51) no back flow prevention device on the mop sink in the rear. Instructed to provide and maintain. (55)the mop is not properly stored when not in use. Instructed to hang mop to properly air dry. Maintain same. Must correct core violations by the date on the report. Priority foundation violation #7-42-090. Citation issued.
