- comments: observed no written employee health policy on the premises. Management instructed to provide a signed employee health policy for each employee. Priority foundation 7-38-010. Citation issued.

- comments: observed no clean up procedure and supplies on the premises for vomit and diarrheal events. Management instructed to provide a clean up procedure and supplies. Priority foundation 7-38-005. Citation issued.

- comments: handwashing signage needed at all dishwashing and prep area handwashing sinks.

- comments: found no thermometer inside the 2-door sandiwch prep cooler. Must provide.

- comments: found bulk food storage containers without labels. Instructed management to label all bulk food storage containers (salt, sugar, flour, etc. ) with common name and maintain.
38. Insects, rodents, & animals not present
- comments: found no recent inspection receipts (at least twice a year per code) on-site at time of inspection, last inspection report inside log book is dated 8/28/20. Instructed to have a pest control log book on site with recent receipts at all times and to have premises serviced. Priority foundation 7-38-020(c). Citation issued.
38. Insects, rodents, & animals not present
- comments: observed pest infestation of over 150 rat droppings, over 150 live small flies, and approximately 15 live ants, and 1 live cockroach throughout facility. Rat droppings observed inside the bottom portion of a display cooler used for storage (underneath the electrical wires/motor), on the floor underneath the wooden bakery prep table, on the floor along the large oven, near the rear exit door, and in the basement on the floor near the front stairs. Observed live small flies in the rear prep area near the dish area and bulk food storage containers, and in the basement by the utility sink. Observed ants on the floor in between broken tiles next to the deli display cooler. Management instructed to clean and remove all rodent droppings from all affected areas and contact pest control to eliminate all pest activity from the premises. Priority 7-38-020(a). Citation issued.

- comments: observed food handling employees preparing food without hair restraints. Management instructed that all food handling employees must wear a hair restraint while preparing food.
48. Warewashing facilities: installed, maintained & used; test strips
- comments: observed facility without a chemical test kit for the sanitizer (bleach) being used on site for dishwashing/sanitizing. Informed person in charge a test kit that accurately measures the concentration in mg/l of sanitizing solution must be provided. Priority foundation violation #7-38-005. Citation issued.
49. Non-food/food contact surfaces clean
- comments: observed dirt, garbage, and old food debris inside the 2-door prep cooler near the flat top that is being used for storage, and inside the display cooler used for storage in the front area next to the front 3-comp sink. Observed an accumulation of grease and dirt on the handwashing sink next to the flat top grill. Must detail clean all areas and maintain.

- comments: found all handwashing sinks slow draining in the prep/dish areas. Observed basement utility sink slow draining with standing water. Must repair all and maintain.

- comments: found a large hole in the wall behind the flour storage containers/wood prep table. Found broken floor tiles near the front deli cooler. Must repair hole and maintain.

- comments: previous core violations from 10/25/2019 report #2320475 not corrected: violation #49 (4-602. 13)detail clean the prep coolers, grease traps, food storage containers, storage shelves, proofer, dough sheeter, deep fryer, ovens, hot holding units, prep tables, sinks throughout all food prep areas. Violation #55 (6-501. 12) clean the walls throughout the premises especially in the food prep areas. Clean the floor throughout the food prep areas. Violation #55 (6-501. 11) replace the broken floor tiles under the sandwich prep cooler in the front prep area. Violation #56 (6-501. 14) clean the grease from the ventilation hood and filters. Must correct previous core violations. Priority foundation 7-42-090. Citation issued.
