- comments: observed no consumer advisory for raw or undercooked foods. Instructed the person in charge to identify such foods by asterisking them to a footnote that states: 'items can be served raw or undercooked or contain undercooked ingredients' and add a statement that reads: 'consuming raw or undercooked animal products may increase your risk of food borne illness, especially if you have medical conditions. 'priority foundation 7-38-005. No citation was issued.
38. Insects, rodents, & animals not present
- comments: observed approximately 20 mouse droppings along the wall underneath the two-compartment sink located in the rear preparation area. Instructed to remove all rodent droppings, clean surfaces, and maintain premises free of pests. Recommended receiving additional pest control service. Priority foundation violation 7-38-020(a). Citation issued.

- comments: observed both toilet room doors not self-closing. Instructed the person in charge to provide a self-closing mechanism for both toilet rooms doors.

- comments: observed grease buildup, food debris, and dirt on the floor under cooking equipment, along walls, and in all corners in prep, and dishwashing, areas. Instructed the person in charge to clean floors under all equipment, along walls, and in all corners in all areas.

- comments: observed one burnt-out lightbulb underneath the hoodedvent, located in the rear preparation area. Instructed the person in charge to replace burnt-out lightbulbs and maintain adequate lighting.

- comments: observed one certified food manager without allergen awareness training. Instructed the person in charge to ensure that any individual that holds a city of chicago foodservice managers certificate obtains allergen awareness training.
