- comments: observed 15 live small flies on walls and ceiling in dish washing area, instructed to clean and sanitize the affected areas, contact pest control for service and maintain. Priority foundation 7-38-020 (a) citation issued.

- comments: observed cutting blade on can opener with dried food debris, instructed to detail clean and maintain.

- comments: observed grease and dried food debris on exterior contact surface of ovens, instructed to detail clean and maintain.

- comments: observed standing waste water in basin of garbage disposal attached to the 3-comp sink, water not draining. Instructed to contact plumber to have basin rodded/repaired. Priority violation 7-38-030(c) citation issued. Given 2 days to repair sink, by 7/23/21

- comments: observed no covered trash can in the unisex staff toilet room. Instructed to provide and maintain.
55. Physical facilities installed, maintained & clean
- comments: observed dirt and debris on floor underneath dish machine and under 3-compartment sink in rear dishwashing area, in all walk in coolers/freezer, and in the chemical storage room. Instructed to clean and maintain.
