- comments: observed no city of chicago certified food service manager nor cityof chicago sanitation certificate on site while tcs food are being prepared and served. Eggs, chicken, pork beef etc. Instructed above that certified food service manager has to be on site with original city of chicago certificate at all time food is prepared and served. Priority foundation violation 7-38-012. Citation issued.

- comments: observed 15 live small flies on walls and ceiling in dish washing area, instructed to clean and sanitize the affected areas, contact pest control for service and maintain. Priority foundation 7-38-020 (a) citation issued.

- comments: observed cutting blade on can opener with dried food debris, instructed to detail clean and maintain.

- comments: observed standing waste water in basin of garbage disposal attached to the 3-comp sink, water not draining. Instructed to contact plumber to have basin rodded/repaired. Priority violation 7-38-030(c) citation issued. Given 2 days to repair sink, by 7/23/21
53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: observed no covered trash can in the unisex staff toilet room. Instructed to provide and maintain.
55. Physical facilities installed, maintained & clean
- comments: observed dirt and debris on floor underneath dish machine and under 3-compartment sink in rear dishwashing area, in all walk in coolers/freezer, and in the chemical storage room. Instructed to clean and maintain.
