- comments: observed person in charge without proof of food safety management training during time of inspection. Must provide.
2. City of chicago food service sanitation certificate
- comments: observed facility did not provide a valid city of chicago food service sanitation certificate for employees working on site. Informed a certified manager with a current city of chicago certificate must be on premises at all times and a valid must remain posted on site whenever time and temperature control for safety(tcs) foods are being prepared, handled and/or served. The certified city of chicago food service sanitation manager, arrived on site at end of inspection priority foundation violation #7-38-012. Citation issued.
10. Adequate handwashing sinks properly supplied and accessible
- comments: observed handsink in prep area and employee restroom not stocked with hand soap. Informed person in charge that hand soap must be available at handsinks at all times. Food handler obtained hand soap for hand sink during time of inspection. Priority foundation violation #7-38-030(c). Citation issued.
10. Adequate handwashing sinks properly supplied and accessible
- comments: "observed no paper towel at the hand-sink in the prep area. Informed person in charge that paper towels must be available at hand sinks at all times. Food handler obtained paper towel for hand sink during time of inspection priority foundation violation #7-38-030(c). Citation combined with above violation
10. Adequate handwashing sinks properly supplied and accessible
- comments: observed no handwashing signage in newly installed handsink in front prep. A sign or poster that notifies food employees to wash their hands shall be provided at all handwashing sinks used by food employees and shall be clearly visible to food employees. Instructed to provide.
24. Time as a public health control; procedures & records
- comments: pizza is out of temperature control at 70°f. The method of time as a public health control does not have an effective written method to record the point in time when the food is removed from temperature control to the discard time. Discussed the haccp concepts and requirement for written procedures with the person in charge. Manager immediately and voluntarily disposed of 45 lbs of products worth $650. 00 through denaturing process. Priority foundation 7-38-005 citation issued.
38. Insects, rodents, & animals not present
- comments: observed pest control logbook with no service reports, invoice was presented at time of inspection. All retail food establishments must maintain a logbook which shall be kept for a period of twelve (12) months and include a copy of the service tickets and the pest control company’s illinois department of public (idph) health license number. Instructed to provide service reports. Priority foundation violation #7-38-020(c).
40. Personal cleanliness
- comments: observed food handlers prepping food without hair restraints. Instructed person in charge that all food handlers must wear a hair restraint.
55. Physical facilities installed, maintained & clean
- comments: observed water stained ceiling tiles at thedish area. Instructed to repair, replace missing panels and/or replace the stained ceiling tiles.
57. All food employees have food handler training
- comments: observed no proof of training for all food handlers on shift during inspection. Instructed to provide and maintain records at all times.
