Report date: May 1, 20205. Procedures for responding to vomiting and diarrheal events
- comments: observed no clean-up policy procedure and items for vomiting and diarrhea. Must provide and maintain. (priority foundation 7-38-005)

- comments: observed no hand washing sign at hand sink at coffee station. Must provide.

- comments: observed no hand washing sink in rear dish washing area. Must install a hand sink with hot and cold running water and maintain. (priority foundation 7-38-030(c)
38. Insects, rodents, & animals not present
- comments: observed evidence of appx. 50 or more mice droppings on floor under pallets and on cardboard shelving unit in walk in cooler,along wall bases and in between clutter in furnace area,on floor under chip display racks. Must remove droppings. Clean and sanitize areas. Recommended to have a pest control operator to service premises on or after 5/1/20. (priority foundation 7-38-020(a)

- comments: observed an appx. '1/2-3/4' gap along right side of rear door. Must make door tight fitting.

- comments: must clean debris build up from walk in cooler fan guard covers.

- comments: must remove excess caulking at basin drains of 3- compartment sink.

- comments: must repair or replace stained ceiling tiles where needed.
