- comments: observed excessive amount of grime and debris on shelving inside walk in cooler. Instructed person in charge to detail clean or replace.

- comments: observed three compartment sink with leak at bottom draining into pans. Instructed person in charge to repair.
55. Physical facilities installed, maintained & clean
- comments: observed items stored on floor inside of non-working walk in cooler. Instructed person in charge to maintain all items at least six inches from floor at all times.
56. Adequate ventilation & lighting; designated areas used
- comments: observed build up of dust on vent in washroom ceiling. Instructed person in charge to clean and maintain.

- comments: observed previous core violations from report #2615823 dated; 4/16/25: #47-rusty surfaces on bottom of prep tables. Instd to remove rust and maintain surface. #53-missing cover for trash receptacle in toilet room. Instd to provide cover and maintain. #53-toilet room door located in rear prep area not self closing. Instd to have door self closing and maintain. #55-3-compartment sink floor drain not clean, debris. Instd to clean and maintain #58-missing allergen training for city of chicago certified manager. Instd to obtain training and maintain verification on site. Instructed person in charge to come into compliance and maintain. Priority foundation #7-42-090. Citation issued.
