- comments: observed no food employee on-site with city of chicago certified food manager training or original certificate while tcs food items (pizza, salad, sandwiches) are being prepared at time of inspection. Instructed to maintain at least one food employee with city of chicago certified food manager training and original certificate posted in public view on-site at all times of food operations. Priority foundation violation. 7-38-012 citation issued.

- comments: observed in-use utensils at the main pizza preparation line stored in still, room-temperature water between uses. Instructed to store in-use utensils on a clean, sanitized surface or under running water between uses and maintain.

- comments: observed the following plumbing fixtures in poor repair at time of complaint inspection: 3-compartment sink drain stopper handle at the wash basin detached and nonfunctional, small leak at drain plumbing beneath rinse basin at the 3-compartment sink, leak at men's washroom handwashing sink drain plumbing (handwashing sink functional). Instructed to repair or replace the listed plumbing fixtures to eliminate leaks and maintain all plumbing fixtures in good repair.

- comments: observed the following core violations from previous complaint re-inspection on 1/13/2026 (inspection no. 2629656) not corrected at time of current complaint inspection: #47 4-501. 11 observed cooler lids at the large pizza prep area line cooler in poor repair, detached from hinges and unable to form complete seal with cooler. Instructed to repair or replace large pizza prep area line cooler lids and hinges and maintain all equipment in good repair and proper adjustment. #51 5-205. 15 observed backflow prevention device at the rear warewashing area utility sink faucet in poor repair, missing cap but functional. Instructed to repair or replace the utility sink backflow prevention device and maintain all plumbing fixtures in good repair. Instructed to correct all core violations by their assigned correct by date and maintain all core violations corrected at all times. Priority foundation violation. 7-42-090 citation issued.
