- comments: observed asterisk missing from some animal derived foods on menu that are served undercooked or cooked to order. Observed no disclosure on menu stating that asterisked animal derived foods are served undercooked or cooked to order. Corrected on site before conclusion of inspection asterisk identifying all animal derived foods that are served undercooked or cooked to order added to menu and disclosure added to menu stating that animal derived foods with an asterisk are served undercooked or cooked to order. Priority foundation violation 7-38-005 no citation issued

- comments: observed no left and right side splash guards on handwashing sink in dishwashing area. Instructed to install left and right splash guards on handwashing sink in dishwashing area to prevent contamination of clean equipment.

- comments: observed milk crates used to elevate eggs in walk in cooler and to elevate small cold holding unit in rear dry storage. Instructed to provide adequate shelving units to elevate all food and nonfood items at least six inches off of floor for ease of cleanability.
