- comments: observed evidence of appx. 40 or more mice droppings inside of motor at unused slushee machine in rear storage area,on floor inside of walk in cooler,under rear storage shelves,on chip display shelf. Must remove droppings clean and sanitize areas. Recommended to have a pest control operator to service premises on or after 1/6/25. (priority foundation 7-38-020(a)(citation issued)

- comments: observed a gap along bottom of front door must make door tight fitting to prevent insect entry.

- comments: must eliminate crates used for food storage in rear storage area, walk in cooler and sales floor and provide adequate shelving units. Must apply a sealant or paint raw wood rear storage shelving units.

Instructed to repair or replace faucet handles and maintain all plumbing fixtures in good repair. #53-5-501. 17 : observed waste receptacle in washroom to be uncovered. Instructed to maintain all washroom waste receptacles covered at all times in unisex restrooms. (priority foundation 7-42-090)(citation issued)
