- comments: still observed evidence of appx. 50 or more live roach activity on premises,around grease trap at 3- compartment sink,cardboard along wall in rear prep area,inside of mopsink and 3- compartment sink ,crawling up wall,inside of cardboard boxes on top of 2- door reach in cooler. Must have a pest control operator to service premises on or after 4/14/26.

- comments: 3-305. 11 inspector comments: observed cardboard boxes stored on floor in between wall and chest freezer and near rear door must elevate and store properly.

- comments: 4-601. 11(c) : must defrost ice build up from both chest freeezers. Must clean food debris build up from prep tables,dough machine,all reach in coolers and freezers,hot holding unit.
