- comments: observed combo sink with the dump sink attached to the back of the hand washing sink in barista station. Chicago department of health does not accept combo sinks, instructed to remove or replace with single exposed sink, according to food code standards. Priority foundation violation #7-38-030(c). No citation issued

- comments: observed milk crates being used as shelving throughout the prep and storage areas. Must remove and provide shelving that is 6" off the floor and provides floor accessibility to ensure adequate facility cleaning.

- comments: observed grease/food debris buildup on cooking equipment, walls, surrounding floors and shelving units in the kitchen/prep areas. Instructed to degrease, clean and sanitize.
