- comments: observed pink and black moldlike substance on shield, interior walls and upper interior of ice machine. Instructed to destroy ice, clean and sanitize interior of ice machine to prevent contamination of ice and maintain. Observed rust on screw on left interior wall holding shield in place in ice machine. Instructed to remove rusty screw and replace to revent contamination of ice in ice machine.

- comments: observed approximately 45 small flies in facility. Observed small fly activity in dishwashing area and in beverage station. Instructed to eliminate small fly activity in facility and clean affected areas, recommended licensed pest control company service facility on or after 6/2/2026. Priority foundation violation 7-38-020(a) citation issued

38. Insects, rodents, & animals not present
- comments: observed no pest control log book on premises, containing record of visits, contract with licensed exterminator and copy of service ticket that sets forth the pest control company's illinois department of health license number. Instructed to provide and maintain on premises pest control log book from licensed pest control company. Priority foundation violation 7-38-020(c) citation issued.

- comments: observed spoons, knives and forks stored laying down in uncovered container on table in beverage station. Instructed to store utensils inverted or covered during storage to prevent contamination.

48. Warewashing facilities: installed, maintained & used; test strips
- comments: observed non working wash gauge on low temperature dish machine. Instructed to repair or replace wash gauge on low temperature dish machine, instructed a ware washing machine shall be equipped with temperature measuring device that indicates the temperature of the water in each wash tank. Priority foundation violation 7-38-005 citation issued.

- comments: observed ice buildup on and behind condensor in walk in freezer. Instructed to remove ice buildup in walk in freezer and maintain.
