- comments: observed no handwashing signage in any washroom. Instructed to obtain handwashing signage and post in all washrooms.

- comments: observed vacuum packaging machine in basement prep area. No haccp plan on site for vacuum packaging machine. Instructed to contact food protection division to apply for haccp plan. Vacuum packaging machine tagged. Priority foundation violation. 7-38-005 citation issued

- comments: observed containers of seasonings in large kitchen prep area and squeeze bottles in small coolers not labeled with common name. Instructed to label food items not in original containers with common name.

- comments: observed two small live roaches crawling on base of wall underneath handwashing sink in between one compartment sink and one door cooler in large prep kitchen area. Observed utility room with hot water tanks with 15 rat dropping inside. Priority foundation violation 7-38-020(a) citation issued.

- comments: observed no covered waste receptable in both womens washrooms. Instructed to provide.

- comments: observed floors in corners and behind equipment with debris and black buildup in kitchen prep and dishwashing areas. Instructed to detail clean and maintain.

- comments: observed dim areas in large kitchen prep area near one compartment and handwashing sink. Instructed to install adequate lighting in kitchen prep area.

- comments: oberved expired food handler training for employees. Instructed to obtain food handler training for all employees and maintain certification on premise at all times.

- comments: previous core violation from inspection report #2567829 dated 12-01-2022. #58- 2-102. 13 observed ceritfied food manger without allergen awareness training certificate. Instructed the person in charge to ensure that any individual that has a city of chicago food sanitation certificate completes allergen awareness training and receives allergen awareness training certificate. Priority foundation violation 7-42-090 citation issued.
