2. City of chicago food service sanitation certificate
- comments: observed no city of chicago food sanitation manager on site while tcs foods are being prepped (such as chicken). Instructed must have a city of chicago food sanitation manager on site at all times when tcs foods are being prepped/served. Instructed must post a valid city of chicago food sanitation manager certification in plain view of customers at all times. Priority foundation violation 7-38-012. Citation issued.

10. Adequate handwashing sinks properly supplied and accessible
- comments: observed no hot running water at customers bathroom handwashing sink due to the knot broken. Temperature of water was 78. 5f at the time of inspection. Instructed to have hot and cold running water at a minimum temperature of 100f at all hand wash sinks. Priority violation 7-38-030(c)

23. Proper date marking and disposition
- comments: observed tcs food item soft serve vanilla mix out of the box and in container in walk in cooler without labels. Instructed to provide discard date labels (not to exceed 7 days) on all foods prepared on site and held for more than 24 hours and maintain. Manager corrected on site priority foundation violation 7-38-005. Citation issued.

- comments: observed a leaky pipe underneath the 3 compartment sink. Must repair and maintain.

- comments: observed debris around dumpster. Instructed to clean and maintain

- comments: observed floors need cleaning along the wall base and in corners to remove spills and debris.

- comments: observed no food handler certificates on site at time of inspection. Instructed all employees must complete food handler training and maintain records thereof on site at all times.
