- comments: 6-301. 14 observed no hand washing sign in the employee toilet rooms. Instructed to provide.

- comments: 6-501. 111 observed evidence of pest activity on site. Observed about 5 live large flies, about 10 live small flies, and about 5 live cockroaches flying around and crawling in the food prep area/dishwashing area. Instructed to eliminate live insect activity, clean and sanitize affected areas, and recommended contacting pest control for a service. Priority foundation. Citation issued 7-38-020(a).

- comments: 6-501. 112 observed dead cockroaches along the wall under the high temperature dish machine and around the floor by the cooking equipment. Instructed to clean and sanitize all.

- comments: 5-205. 15 observed hot water pressure needs to be increased at the right faucet located at the 3 compartment sink. Instructed to repair.

- comments: 5-501. 15 observed excessive garbage on the ground around the outside dumpsters (old furniture, crates etc). Instructed to remove excessive garbage and maintain the outside dumpster area at all times. Priority foundation. Citation issued 7-38-020(b).

- comments: 6-501. 12 observed excessive dirt and food debris on the floor in the food prep area behind refrigeration units and under the high temperature dish machine. Instructed to clean along the walls and in all corners.
