- comments: violation codes: 6-301. 14 inspector comments: observed no hand washing signs in 2nd fl. Boy's & 1st fl. Staff must provide and maintain.

Sink must be large enough to submerge the largest piece of equipment(hotel size pans). Must install and maintain. (priority foundation 7-38-025)

- comments: still observed black foul smelling waste water backing up from floor drain and pooling across the floor in 1st fl. Toilet room when water is turned on in sink and toilet flushed. Must remove waste water,clean and sanitize area. Repair and maintain. (priority 7-38-030(c)

- comments: violation codes: 5-501. 15 inspector comments: observed outside garbage dumpsters in poor repair missing rear and pushed in lids in center making dumpster not tight fitting,trash and debris on ground in corral. Must repair or replace lids to be tight fitting, remove trash and maintain. (priority foundation 7-38-020(b)
