- comments: observed water leaking onto floor from toilet when toilet is flushed in staff toilet room on 1st floor near kitchen area. Toilet room had strong odor throughout. Instructed manager to repair toilet. Toilet room tagged priority violation 7-38-030(c) no citation issued.
